Introduction: Combined pulmonary fibrosis and emphysema (CPFE) is a distinct under-recognized lung disease characterized by upper lobe emphysema and lower lobe pulmonary fibrosis.(1) Both emphysema and idiopathic pulmonary fibrosis (IPF) are described conditions associated with the development of lung cancer. (2) Lung cancer has been identified in 4.4% of IPF cases with the annual incidence of lung cancer arising in chronic obstructive lung disease previously reported at approximately 1%. (3, 4)
