Introduction: Continuous Positive Airway Pressure (CPAP) treats Obstructive Sleep Apnea (OSA) effectively but compliance is poor. Interface type may impact long term compliance, with oronasal (ON) being associated with adverse effects and reduced compliance. Extended trial of interface before deciding on a preferred interface may improve compliance.

Method: Patients with newly diagnosed moderate-severe OSA (Apnea-Hypopnea Index (AHI)>15) had autoPAP trial of 3 interfaces (nasal mask (NM), ON, nasal pillow (NP)) in randomised order for 1month each before continuing on their preferred interface for months 3-6. Compliance was defined as CPAP use >4h/night and was reassessed at 6thmonth. Satisfaction was assessed by Visual Analogue Scale of 0(very happy) to 10(very unhappy).
