INTRODUCTION: Diffuse idiopathic pulmonary neuroendocrine cell hyperplasia (DIPNECH) is an extremely rare pulmonary disease with abnormal proliferation of neuroendocrine cells. It is being identified with higher incidence due to enhanced awareness, imaging and histopathology. It is recognized as a precursor to peripheral carcinoid tumors and can lead to progressive obstructive pulmonary disease. It is most common in middle-aged, nonsmoking females presenting with obstructive symptoms including cough and dyspnea.
