INTRODUCTION: Idiopathic systemic capillary leak syndrome (SCLS) is a rare and sometimes fatal condition characterized by recurrent episodes of hypotension, hypoalbuminemia, and hemoconcentration.1 Patients require aggressive hemodynamic and respiratory support, and can easily be mistakenly diagnosed with sepsis or anaphylaxis. We describe a case of SCLS presenting as recurrent acute respiratory failure with peripheral edema mimicking episodes of pneumonia with sepsis.
