INTRODUCTION: The incidental pulmonary nodule is a common radiologic finding. Although follow up guideline was published by Fleischner Society in 2005, there have been significant shortcomings. Follow-up of ground glass nodules (GGN) in particular was a source of considerable confusion. Ground-glass opacities are often the earliest radiologic manifestation of adenocarcinoma of the lung. We present a case of invasive adenocarcinoma that presented as GGN and over a period of 20 years exhibited very unusual growth pattern.
