INTRODUCTION: Small cell lung cancer is typically diagnosed with lung mass or paraneoplastic syndrome. Syncope is a common presentation; however malignancy is rarely considered as a differential diagnosis. We present an unusual case of syncope due to compressive right ventricle outflow obstruction secondary to an extra cardiac mass.
