INTRODUCTION: Pneumothorax (PTX) & safe time to air travel has been an area of uncertainty. Commercial flight induces physiological stress due to low atmospheric pressure causing gas expansion within enclosed space based on Boyle’s law1. We present a case of bilateral iatrogenic PTX with eventful flight leading to hypoxia & subsequent bilateral Hydro-PTX on presentation.
