PURPOSE: In 2012 the American Association of Blood Banks (AABB) developed a clinical practice guideline for red blood cell transfusion. It recommends following a restrictive transfusion strategy (Hemoglobin (Hb) of 7 g/dL) in stable patients. The guidelines, based on data from randomized control trials, focuses on using hemoglobin concentration thresholds and clinical variables in decisions to transfuse. At our institution, adherence to the guidelines was poor. Our goal was to improve adherence by using the EHR (Epic) and creating a Best Practice Alert (BPA) to educate physicians about the guidelines and to encourage the use of a restrictive red blood cell transfusion strategy.
