INTRODUCTION: Neuroendocrine small cell carcinoma (SCC) usually presents as a hilar mass with bulky mediastinal lymphadenopathy. Other rare pulmonary neoplasms include large cell neuroendocrine carcinoma (LCNEC) that generally presents as mass in periphery of the lung parenchyma. We present an interesting case of primary mixed small and large cell neuroendocrine carcinoma of the mediastinum. To our knowledge, this has never been reported in the past.
