INTRODUCTION: Adalimumab (TNF Inhibitor) has been available since 2008 for the treatment of various autoimmune diseases. Despite clinical efficacy, many TNF inhibitors have a considerable side effect profile. There have been numerous case reports outlining the pulmonary side effects of adalimumab including, but not limited to, interstitial pneumonia, fibrosing alveolitis, and granulomatous pneumonitis. Here we present a case of a small size vessel pulmonary vasculitis attributed to adalimumab. We believe it is the first such case described.
