INTRODUCTION: Tumor lysis syndrome (TLS) is an oncological emergency characterized by a combination of metabolic derangements from rapid turnover from cell death in certain cancers. It is usually seen during the initiation of chemotherapy. TLS occurring spontaneously, without initiation of chemotherapy, is rare and its occurrence in solid tumors is rarer. We report a case of spontaneous TLS in a patient with a uterine leiomyosarcoma. Such a case has never been reported before.
