INTRODUCTION: Pneumocystis pneumonia (PCP) is a known complication of chronic glucocorticoid use. Suppression of cell-mediated immunity and alterations in lung surfactant have been suggested as mechanisms through which glucocorticoids predispose to the development of PCP. We present a case of PCP in a non-HIV infected patient who presented with rapidly progressive hypoxemia within hours of hospital admission.
