INTRODUCTION: Acute respiratory failure due to primary pulmonary Kaposis Sarcoma is rare in the post highly active antiretroviral therapy (HARRT) era. We describe a patient who presented with acute respiratory failure and worsening radiographic appearance despite antibiotic therapy and found to have Kaposis sarcoma of the lung without involvement of the skin
