Analysis for Nineteen Pulmonary Cryptococosis Patients of Non-immundeficiency as Diagnosed FREE TO VIEW

PURPOSE: To discuss the clinical characteristics of non-immundeficiency pulmonary cryptococosis and its diagnostic methods, in order to conduct the right treatment methods and sovle the disease effectively, to decrease misdiagnosis rate.

METHODS: The clinical data of 19 pulmonary cryptococcosis cases of non immune deficiency patients who have been treated in our hospital in 5 years were analyzed retrospectively, and the related literature was discussed.

RESULTS: ​The main clinical manifestations were: cough (12/19, 63.2%), fever (7/19, 36.8%), expectoration (5/19, 26.3%). Three cases (26.3%%) were a symptomatic. The X-ray and chest CT showed solitary or multifocal masses or no dules (14/19, 73.7%). Fifteen cases (78.9%) were misdiagnosed, including 4 (21.1%) were misdiagnosed as common bacteria infection, 6 (31.6%) as pulmonary tuberculosis, 5 (26.3%) as lung cancer, 4 (21.1%) as pneumonia. Six patients received surgical resection. Two patient suffered from cryptococcal meningitis after operation. Twelve patients received anti-fungalagents. Nine patients were cured ormarkedly improved.

CONCLUSIONS: Pulmonary cryptococcosis in immunocompetent population can occur, pulmonary cryptococcosis with imaging manifestation have a great diversity intype, so it is difficult to diagnose, needs to be confirmed by pathology and pathogenic examination; fluconazole on curative effect of the treatment of such patients is significantly, less adverse reaction.

CLINICAL IMPLICATIONS: Through the retrospective analysis of the data, In order to make medical workers setup awareness of the non immune deficiency patients with pulmonary disease, make the patients get early detection and early active and effective treatment, thereby reducing the misdiagnosis and false treatment.
