Asbestos-Related Chest Diseases After Occupational Exposure- A Never Ending Story FREE TO VIEW

PURPOSE: An over time evaluation of asbestos-related lung diseases in former workers with industrial occupational exposure to asbestos fibers was performed.

METHODS: A prospective survey of consecutive occupational asbestos exposure diseases, presenting to Occupational Medicine Department of Colentina Hospital from Bucharest, Romania, over a 3-year period (from January 2011 to December 2013), was carried out. Subjects were considered all inpatients having exposure history to asbestos fibers and first evaluation of asbestos-related diseases (ARDs). Screening of ARDs included an evaluation of duration of specific occupation and a further estimation of retention time, according with the moment of first diagnosis of disease. Smoking, demographics, clinical, spirometry and radiologic details were also recorded and analysed. Retention time of asbestos fibers was considered from the first moment of exposure till the moment of hospitalization and evaluation of each patient. Since 2007, asbestos exposure has been prohibited in Romania.

RESULTS: 110 inpatients, mean aged 59.32 year-old, with male predominance (sex ratio of 1.3 M/ 1 F) were identified as former asbestos cement workers. The average of occupational exposure time was 18.72 years (limits: 2.5- 41 yrs). The average of retention time was calculated to 35.32 years (limits: 9-52 yrs). Smoking was denied by 87 subjects (79%). ARDs were diagnosed for the first time in 80% of cases (n=88/110), consisting in pulmonary asbestosis (59%), pleural plaques with asbestosis (14%), diffuse pleural thickening (3%), malignant mezothelioma (2%), and lung cancer (2%). The prevalence of bronchitis and COPD in never smokers was high (n=35/87; 40.22%).

CONCLUSIONS: The great majority of asbestos exposed workers is susceptible to develop asbestos-related diseases. Malignant complications of exposure to asbestos occured in 4% of patients.

CLINICAL IMPLICATIONS: Occupational history must be an important element of a medical interview for preventing the delayed diagnosis of asbestos related diseases.
