We thank Dr Sperandeo and colleagues for their comments on our study published in a recent issue of CHEST.1 We agree with their assessment that thoracic ultrasound is underused from a diagnostic perspective. Despite a growing evidence base for use of ultrasound-guided pleural biopsy in diagnosing malignant disease, current international guidelines at present do not reflect this,2 and there is no clear consensus agreement on how these techniques should be incorporated into the diagnostic pathway. Physician-led ultrasound-guided pleural procedures represent an exciting emerging area, with further research and consensus opinion required to define the optimum strategy for investigating potential malignant pleural disease and how best to incorporate these procedures into training programs for the respiratory specialist.
