In this issue of CHEST (see page 390), Acuña-Izcaray and colleagues1 have applied the well-validated Appraisal of Guidelines for Research and Evaluation (AGREE) II instrument to a set of asthma guidelines published by prestigious organizations in well-recognized journals in the past decade and demonstrated striking deficiencies. Their results are consistent with previous evaluations that used the AGREE instrument and with a study that looked at 130 guidelines selected at random from the National Guideline Clearinghouse that demonstrated poor compliance with Institute of Medicine standards.2
