EVALUATION OF EFFICACY OF DISCRIMINATION OF PLEURAL TRANSUDATES AND EXUDATES BY MEASURING SPECIFIC GRAVITY OF PLEURAL EFFUSION USING REFRACTOMETER FREE TO VIEW

Abstract

PURPOSE: To evaluate efficacy of discrimination of pleural transudates and exudates by measuring specific gravity(SG)of pleural effusion using refractometer.

METHODS: we included 112 patients (calculated sample size) with pleural effusion admitted to Imam Khomeinie hospital.Quality of pleural effusion evaluated with Light criteria and specific gravity of it tested with refractometer.

RESULTS: 66.1% of effusions were exudate and 38.9% were transudate and mean of SG between transudates and exudates were significantly different(P<0.05).Sensitivity and specificity of measuring SG of pleural effusion were 70.2% and 68.4% respectively in cutoff level SG>1.024 .Area under ROC curve was 0.709(95% CI, 0.606 and 0.812).

CONCLUSION: This test is fast and simple and our results shows that its sensitivity and specificity is in an acceptable level.

CLINICAL IMPLICATIONS: we can use this test in clinic and immediatly make decision for pleural biopsy.
