PURPOSE:  Chronic obstructive pulmonary disease (COPD) is one of the leading cause of death worldwide. COPD is characterized by chronic inflammatory process that is evident in the pulmonary compartment and in the circulation. The presence of systematic inflammatory response may influence quality of life due to weight loss, muscles wasting and tissue depletion. This study was done to evaluate the concentrations of C-reactive protein and prealbumin in stable patients to look for any correlation of BMI with disease severity.

METHODS:  Fifty two stable patients mean age 59.35 ±11.48 with moderate to very severe COPD were enrolled. Concentrations of serum C-reactive protein and prealbumin were measured by latex enhanced immunoturbidimetry and quantitative turbidimetric assay respectively. Anthropometry including BMI was done in all patients. Written informed consent was obtained. The study was approved by ethics committee of our hospital.

RESULTS:  13 patients had moderate ( FEV1 64.53±12.12% predicted), 16 severe(FEV1 38.31 ±6.16 % predicted) and 23 very severe (FEV1 23.91±5.99% predicted)COPD. Median (interquartile range) levels of CRP in moderate, severe and very severe COPD was 0.30(0.20–0.60), 0.50(0.30–1.65), 0.70(0.40–1.60) mg/dl respectively. Median (interquartile range) levels of prealbumin in moderate, severe and very severe COPD was 15.70(11.41–19.72), 10.79(9.62–14.04), 13.60(9.94–15.83) respectively. Significant difference was found in the CRP and prealbumin levels in the 3 groups. Mean BMI in patients with moderate COPD was 21.77±2.70, severe 19.11±3.18, and very severe 18.25±3.01 kg/m2.The difference of BMI in the three groups was significant (P<0.006).

CONCLUSION:  In stable COPD, CRP levels increased with severity of disease and prealbumin level declined. BMI also showed a significant decline as disease severity increased.

CLINICAL IMPLICATIONS:  In COPD, as the disease progresses inflammation increases and an associated decline in BMI occurs. Aggressive treatment of systemic inflammation may help in improving BMI and quality of life.
