COMPARISON OF THREE SIZES OF POLYETHYLENE GLYCOL MODIFIED ALBUMIN TO ALBUMIN IN SEPTIC SHOCK IN THE RAT FREE TO VIEW

Abstract

PURPOSE: It is reported that polyethylene-glycol modified albumin- 5000 Daltons (PEG-Alb) has superior plasma expansion properties compared to albumin and crystalloid. The effect of different size PEG (20.000 and 40.000) on plasma expansion is not known.

METHODS: We compared albumin plasma expansion properties {Blood pressure (BP), hemodilution (Htc), colloid osmotic pressure (COP)} to those of PEG-Alb 5000, PEG-Alb 20.000 and PEG-Alb 40.000 in a cecal ligation and puncture model of sepsis (CLP) (see figures).

RESULTS: PEG-Alb treated groups performed better than albumin or saline.PEG-Alb 40.000 had the most hemodilution and increase in COP and maintenance of BP. PEG-Alb 20.000(one site attached) performed better than albumin but worse than the PEG-Alb 5000 (6 sites attached) or PEG-Alb 40.000 (one site attached).

CONCLUSION: We report evidence supporting the contention that the superior resuscitation properties of PEG-Alb in sepsis (or capillary leak conditions) are derived from improved intravascular retention related to the molecular weight of the compound.

CLINICAL IMPLICATIONS: Minimizing capillary leak in Systemic inflammatory conditions will help decrease the severity of multiorgan dysfunction syndrome and hence morbidity and mortality.
