PURPOSE: The introduction of the different techniques to explore changes in the mucosa (life system, autofluoresence) had an impact in the diagnosis of the lesions that were very difficult to see. In Gastroenterology Spectral Images (SI) has had an especial influence in the diagnosis, and interpretation on site, of different lesions. The Fuji Intelligent Chromo Endoscopy (FICE) from FujiFilm has been proven in GI with excellent results.We consider that this technology has promissory influence in other fields like bronchoscopy.
METHODS: We perform video endoscopy in 8 patients (Bronchoscope EB-470T) and apply the FICE system integrated (4400 processor); 6 patients had suspiction of lung cancer, 2 of them had undergone lobectomy for lung cancer. We made in real time different FICE filters (SI 1-9) to explore the differences in reflectance. We used the same filters in all the patients and obtain still images in all studies for comparison.
RESULTS: The group with suspicious lung cancer (6 patients), 4 patients had endoscopic findings. In 2 of them the lesion was not totally evident with White Light Bronchoscopy (WLB). With FICE was more evident, and we found abnormality in the mucosal vascularity. The biopsies were positive for adenocarcinoma.The 2 patients with lobectomy: one had visual changes in the WLB, but with FICE technique the changes were bigger than we thought. The biopsy was positive for recurrence of cancer.
CONCLUSION: The use of FICE can help the endoscopist to enhance the description's range of lesions at the airway, show vascular abnormalities, and increase accuracy obtaining biopsy samples. The application in real time is excellent. The processor has no delay between filters. This bronchoscope can easily be use for therapeutic purposes.
CLINICAL IMPLICATIONS: The SI-FICE brings a new way to identify different lesions at bronchoscopy. This application can help to recognize malignant and premalignant lesions with accuracy. This advanced technology allows an increased correlation between the images processed and the histological results. Further studies are needed to demonstrate all the possibilities of virtual chromoendoscopy.
