CHEST covers many areas. Some topics are directly relevant to clinical care and the practicing physician. Others may be most concerned with health-care delivery and financing. Regulatory issues affect almost everything we do, from patient care, to reimbursement, to clinical research. The potential for conflict of interest is all too common in our daily activities. Research, whether basic, clinical, or translational, is changing our lives. In this series, we hope to include these diverse areas and to capture their anticipated directions and effects on us as pulmonary, critical care medicine, and sleep physicians. The goal of each article is to present the current state of the art and then the directions in which the field may be headed, interpreted broadly to include areas relevant to the readership of CHEST. Where does an expert think the field is heading and where should it be heading? How might these changes, at the local level, affect you, your colleagues, coworkers, and patients? How might these changes affect the field globally?
