Neck Circumference: A Predictor of Sleep Apnea Syndrome in Male FREE TO VIEW

PURPOSE: The analysis of neck circumference (NC) in patients with sleep apneea syndrome (SAS) and comorbidities.

METHODS: Nine hundred sixty eight consecutive patients evaluated between June 2005 and June 2009 for SAS, completed Epworth questionnaire, different comorbidities, body mass index (BMI), neck circumference (NC), somnography and polysomnography for apnea -hypopnea index (AHI). We evaluated Odds Ratio (OR) with 95% confidence interval (CI) in a univariate analysis. Multiple linear regression was used to identify independent predictors.

RESULTS: 727 males (75,1%), 241 females (24,%), age 52 ±11,89 years, BMI 32,06 ±5,22 kg/m2, AHI 32,10 ± 24,41/h. In 73% of male NC ≥ 43 cm, in 42% of females ≥ 41 cm. NC in univariate analysis, OR 1,15, CI 1,07-1,22, p< 0,001. NC in male, OR 4, 243, CI 2,10-8,53, p< 0,001. In female, OR 1,40, CI 0,51-3,83, p = 0,511. Regarding AHI (≤ sau ≥ 10/h) NC was 41,62 ± 4,27 cm vs. 44,37 ±4,69cm, CI 95% 40,77-42,46 vs. 43,86-44,17, p < 0,001, in systemic hypertension (42,98 ± 4,84 cm, 95% CI 42, 26- 46,25, vs. 44,24 ± 4, 59, CI 44,81 - 47,36, p = 0,0068 ), in COPD (45,07 cm vs. de 43,45 cm, p = 0,0091). In multiple regression analysis, NC - AHI, r 0,401, p< 0,001. Together with desaturation index, Epworth score and obstructive apneas, relation NC - AHI, r = 0, 79, p< 0,001.

CONCLUSION: Neck circumference is strong predictor for OSA at male and is significant higher at patients with SAS, systemic hypertension, COPD. In female other factors influence the diagnostic of SAS.

CLINICAL IMPLICATIONS: A simple measurement can predict sleep apnea syndrom in male.
