A 30-year-old man in whom HIV infection had been diagnosed 2 years previously was admitted to our hospital with a history of fever, cough, and fatigue for 3 weeks. Two years ago, when the HIV infection had been diagnosed, he was started on highly active antiretroviral therapy (HAART), with questionable adherence to therapy. He never experienced an AIDS-defining illness or opportunistic infection. Two months prior to this hospital admission, antiretroviral therapy (with tenofovir, emtricitabine, atazanavir, and ritonavir) was reinstituted for an absolute CD4 count of 65 cells/μL and a viral load of > 100,000 copies RNA per milliliter
