A 58-year-old African-American woman with hypertension, gastroesophageal reflux, and a remote history of recurrent deep venous thrombosis presented with cough, subjective fevers, dyspnea, and vague left upper abdominal pain for several days. She was a nonsmoker, and the family history was unremarkable. Her medications were warfarin sodium (Coumadin; Dupont; Wilmington, DE), omeprazole (Prilosec; AstraZeneca; Whitehouse Station, NJ), and hydrochlorothiazide (HCTZ; Zenicagoldline; Miami, FL).
