In the January 2001 supplement Sixth ACCP Consensus Conference on Antithrombotic Therapy, the article “Intravenous Thrombolysis in Acute Myocardial Infarction” (CHEST 2001; 119:253S-277S), by Ohman et al, contained three errors.

On page 271S, section 2.1.3 should read as follows: “For patients receiving alteplase (grade 1B), r-PA (grade 1C), or TNK-tPA (grade 1C), we recommend administration of IV unfractionated heparin for 48 h.”

On the same page, section 2.1.4 should read as follows:“ For patients receiving IV heparin with alteplase, r-PA, or TNK-tPA, we recommend administration of either standard-dose unfractionated heparin (5,000-U bolus followed by 1,000 U/h) (grade 1C) or weight-adjusted dosing (60-U/kg bolus [4,000 U maximum] followed by 12 U/kg/h [1,000 U/h maximum]) (grade 2C), both adjusted to maintain an APTT of 50 to 70 s.”

Again on page 271S, section 2.2.1 should read as follows: “For patients with known or suspected heparin-induced thrombocytopenia or thrombosis who are receiving fibrinolytic therapy (either alteplase or streptokinase), we recommend administration of IV hirudin (lepirudin 0.1-mg/kg bolus followed by 0.15 mg/kg/h infusion) (grade 2A).”
