PURPOSE: Sarcoidosis is multisystem granulomatous disorders with chronic course without aetiological agent. Lung involvement are the most common manifestations. Health practice require additional methods in obtaining all organ involved.What is role of 18F FDG PET scanning in sarcoidosis?
METHODS: Prospective study on sarcoidosis patients with chronic course.Previously, sarcoidosis were obtained -ACCES group with histological confirmations.
RESULTS: We analysed the obtaining results from 31 patients with chronic course of sarcoidosis were obtained ( 18 F/ 13 M, average years of 43.7; mean level of ACE: 57,32U/L, UCa/24h:4.25mmol/24h).The main symptoms were fatigue,dyspnea and chest discomfort. Radiographic stage of pulmonary sarcoidosis : III - 19 pts, II- 10pts, IV-2pts.Nonpulmonary involvement were obtained in 19 pts. A total of 31 of the 188 FDG PET scans revealed hypermetabolism in the 30 patients. FDG PET scan positive pulmonary findings were in 29 patients.Hypermetabolism in lymph nodes ( mediastinal and extrathoracis) and positive pulmonary scan were obtained in 21 patients.Positive pet scan findings on extrathoracis organ were obtained in 5 patients. Pulmonary positive pet scan findings were obtained in 8 pts. New, nonpulmonary localized positive pet scan findings were obtained in: cortex of central nervous system-2 females, muscles in 2 males, myocardium in 2 males, glandular thyroidal in 2 females,skeletal in 3 males. Levels of SUV varies from 3.2 do 21.
CONCLUSIONS: In our, expensive, but worth analyse,positive pet scan findings can suggest the activity of sarcoidosis and finding new organs involved with sarcoidosis.
CLINICAL IMPLICATIONS: FDG PET scan -diagnostic tool and parameter of sarcoidosis activity.
